Our Mission
CareWave Solutions SWVA LLC was founded with the mission of providing high‑quality Consumer Directed Service Facilitation services throughout Southwest Virginia.
We believe individuals and families should have the ability to direct their own care while receiving the guidance and support needed to navigate Medicaid waiver programs. Our role is to help ensure services are implemented safely, effectively, and in alignment with each individual’s goals.
Our team is dedicated to maintaining open communication with individuals, families, support coordinators, and attendants to promote successful outcomes and long‑term stability.
What We Do
CareWave Solutions SWVA LLC provides Consumer Directed Service Facilitation (CDSF) services for individuals enrolled in Virginia Medicaid waiver programs.
Our role is to support individuals who choose to direct their own services by helping them manage responsibilities related to hiring attendants, maintaining service compliance, and ensuring the health and safety of the individual receiving services.
We work closely with the Employer of Record (EOR), families, and support coordinators to ensure services remain aligned with the Individual Support Plan (ISP).
Service Offered
Our Consumer Directed Service Facilitation services include:
• Initial service setup and orientation
• Ongoing service monitoring and oversight
• Assistance with Employer of Record responsibilities
• Monthly contact and support
• Quarterly service reviews
• Health and safety monitoring
• Documentation and compliance support
• Guidance on backup plans and emergency procedures
• Coordination with support coordinators and care teams
Our goal is to ensure individuals receive the services they need while maintaining full compliance with Virginia Medicaid requirements.

• Community Living (CL) Waiver
